All policies must cover: doctors, hospitalizations, prescription drugs, outpatient services/tests, preventive services/tests (with no copays), emergency, maternity and pediatric services, rehabilitative services/devices and mental health/substance abuse. Policy premiums will be available October 1 when you can see if you qualify for premium support (100% to 400% of the federal poverty level--family of four earning up to $94,000/year; individuals up to approximately $45,000). No pre-existing condition exclusions. No annual or lifetime coverage limits. Annual and lifetime caps on out-of-pocket costs.
