      Medicare pays claims as submitted, even claims for treatment of dead people- nobody checks. This results in considerable loss, although nobody knows exactly how much. They also pay for useless treatments, I have seen that in my own patients, and for ridiculous Medicare Advantage perks (I was on Medicare Advantage).

      I suggest that you read some of the work by Prof Malcolm Sparrow about automatic dispersal of government funds on trust, without any human at any competence level checking. You can also check took advantage of mother & Medicare Money lost to fraud and encouraging crackpot treatments are costs.

        and there are people out there who teach providers how to "maximize" their billing for Medicare, i.e., code appropriately for an unneeded test, because there is insufficient oversight. As long as the codes match, Medicare will pay.

        But in order to have more oversight, guess what? Overhead will go up. Is there a cost savings to be had? Depends on the magnitude of the possible overbilling and fraud (because that's what that is.)
