    "government official" is a more restrictive definition than "Anyone being paid by the public".  You can receive payment for services out of public funds without being a "government official" like a police officer.

    Moreover, this refers to a "cardinal First Amendment interest", that is, a settled concept that has long held a carve-out for physicians in their work.  As this ruling doesn't address the expectation of privacy of physicians paid by "the public" (it had no reason to, of course) I can't make the same guesses that you are making.

    It seems curiosity has killed the cat that had my tongue.
