    apply to the self-insured if it isn't assisted by a 3rd-party?

      but as I said most do because of employee relations issues.

      And as an ex-benefits manager, I don't know of any company that handles administration in-house. It's too complex a and opens them to liability. By employing insurers to administer the plan they can remain neutral in decisions regarding approval and denial of services.

      Plus it would be costly in terms of staffing and claims processing software, etc. to do this in-house.
