but in my experience there was a clear difference between the mean-spirited hard-core sexist jackasses in my formerly male-dominated profession and the guys who were genuinely paralyzed sometimes by not knowing whether they were supposed to open a door for me or not.

Do you think there might be a similar distinction between people who are capable of respecting and accepting gays as equals as soon as they overcome whatever homophobia they might have grown up with, and people who embrace homophobia as just another way to express what's in their hate-filled hearts?

but maybe this, from the midday open thread, can help:

Homophobia is more pronounced in individuals with an unacknowledged attraction to the same sex and who grew up with authoritarian parents who forbade such desires ... Individuals who identify as straight but in psychological tests show a strong attraction to the same sex may be threatened by gays and lesbians because homosexuals remind them of similar tendencies within themselves ... In many cases these are people who are at war with themselves and they are turning this internal conflict outward.
I think that as long as the homophobia is externally generated it can be overcome.  The other people, well, I'm not sure we can do anything about them beside letting history take care of things.

All it takes is security in your own civil rights to make you complacent.
