    No one group is responsible for this mess. Everybody is pushing in a different direction on some particular part of a broken system bigger than any of them: patients, doctors, nurses, hospitals, insurance companies, politicians, drug companies, equipment manufacturers, lawyers, churches, foundations, Super-PACs...Now we have the beginnings of a system in which many constituencies, but by no means all, might well push in the same direction on things that nearly work in order to make them better.

      are moving on and up. They are now opening pharmacies within their practices. There is evidence that some doctors are basically on the payrolls of some drug makers so this will lead to more prescribing for fun and profit rather than best interests of patients.

      You are right when you speak about systems. For the most part our health care is a system of interlocking systems that is almost beyond diagraming. It is certainly beyond force or for the most part coercion, central planning or simple prescriptions.

      This law has many carrots and sticks but they will get nowhere without a stronger emphasis on education both within and without the system(s).
