    They make money by delaying valid claims. Some people just give up (so the insurance company gets to keep the money). But if you keep pestering them and they do end up paying out, they've been able to earn interest on the investment of the money in the meantime.

    Often it's something as simple as changing the coding to something different, and the claim goes through just like that. Poof.

      I didn't put in many claims - but EVERY ONE OF THEM was DENIED before it was paid. I had to call multiple people every single time and argue - "No, it says on page 46 that you DO cover that!" "Hm, I don't have that in my book here...." "Page 46, paragraph 8..." "Oh, hmm....I guess we do..."

      These weren't claims that were going to break their budget; we're talking about amounts like $50-$100. Each claim probably took me an hour or more on the phone to get them to cover. Crazy.
