That's part of how physicians in a particular region can control the range of reproductive health services available to women. It's pretty easy for OB/GYNs, for example, to make sure that none of their colleagues (or family practice docs) make a practice of attending home births-- set up a clause about the number of deliveries the doc has to attend per year, or range of services they have to offer, or something, that would be impossible for an independent practitioner to maintain.

I was told more than once when I worked in Louisiana that the hospital that employed me had been built to accommodate a particular physician group. It was essentially "their" hospital. I don't think members of that partnership were going to restrict themselves out of practice.

My big issues with the use of unconfirmed "tubal" pregnancies as a route to termination were twofold: One, the hypocrisy-- The community didn't want any overt abortion services, but still wanted to have abortion as an option. Two, the lack of consideration for the nurses administering the drug-- There are conscience clauses for a reason. We weren't working in a setting where participating in elective termination of pregnancy was part of the job description. This back-door arrangement took away our professional autonomy to exercise our own ethical reasoning. Several of my colleagues were devout observant Catholics; this was a shitty thing to do to them.
