    The Republicans want "concessions" in return for some kind of budget deal.

    And it's pretty clear from this article as well as others that a major goal of health reform is to ensure that people are fully covered to at least three times the poverty rate.  Why not just make the "concession" out of provisions that make the benefit gradually roll off between, say, 300 percent and 500 percent of the poverty line (that can be the opening negotiating position) and 300 and 425 percent of the poverty line (that can be the fallback).

    It certainly needs to be addressed, and soon.  And this upcoming "fiscal cliff" is an opportunity to do it.
