    Different questions have different purposes:

    Age (might mean they recommend a prostate exam or mammogram)
    Chicken pox (might have bearing on whether you have shingles)
    Insurance coverage (who will they bill?)

    Why are they asking about race of the patient? I suspect, but don’t know for sure, that it’s for statistical reports – like how many African-Americans come to the Emergency Room, how many whites die of cancer, and so on. Yes, there are some diseases (Tay-Sachs or sickle cell anemia) that correlate to ethnic groups or races, but I think it’s probably a statistical thing.

    It might also be a bureaucratic holdover from the past. Maybe 20 years ago (or 50 years or whatever), someone decided to include race on the hospital form and then later, when they updated the form, it just stayed there.
