    bishops are supposed to be consecrated (ordained) by the laying on of hands by other bishops who can trace themselves back to the original 12 Apostles.  This is the principle of Apostolic Succession, which exists in the Roman Catholic Church, the Orthodox Churches, the Oriental Churches, and the Protestant Episcopal Church of the United States, which was its official name.

    The last is interesing, because to be a Bishop in the Church of England one had to swear loyalty to the monarch  - remember bishops in the C of E sit in the House of Lords ex officio.

    The first bishop of the Episcopal Church after the American Revolution, Samuel Seabury, obtained his episcopal ordination through Scottish "Non-Juring" bishops - descended from an apostolic line which did not swear allegiance to an English monarch.

    Of course, there are many groups for which the leaders claim the title of bishop which lack apostolic succession, but if you want to be picky they cannot claim the same legitimacy and connection with early Christianity.
