    Diagnostic principle: If the set of symptoms and circumstances you're looking at match a very common disease state and maybe a few very rare disease states, it's probably the common one. Treat it as if it is and see what happens.

    If the treatment for the common disease state works, you were right. If it doesn't, then you can start looking at the weird possibilities.

    Unfortunately, far too often the response when the common treatments fail is instead to blame the patient for not following your instructions and assume s/he is lying about something.

      And you are obviously correct.  I would add that sometimes (in my experience) MD seems to mean medical deity to the physician him/herself and that's the problem stated in your last sentence.  I could elaborate, but ...
