    I'm a retired fire service Captain/EMT. The standard for transporting a person to the ER against their will here in Utah was the opinion of the highest medical rank on-scene (Paramedic or EMT) that the patient was "dangerous to themselves or others."

    If we arrived at that decision, we took the patient by ambulance to an ER. If the ER doc concurred, the patient would be transferred for appropriate psych treatment regardless of their opinion.

    If you were to call 911 during a manic episode, your local EMS system might make incident to ER to inpatient facility possible.

    When the cops show up with the EMS, try to assure she ends up with the EMS people; discuss her relevant medical history to make an it an EMS incident rather than something the police should handle.

    Paramedics and doctors are preferable to police, lawyers, and judges.
