    under guidance (read heavy hand) of the federal government.

    Some of that will change under ACA. For instance, right now, states can determine what level of income qualifies for Medicaid, and as you'd suspect, red states are far more draconian than blues ones, generally speaking.

    In Texas, for a family of 3, income must be less than 26%  of the federal poverty level ($19,530) or $5,077 per year to qualify for Medicaid. Yes, if you earn $5,078 to support your family of 3 in Texas, you're on your own for health care - but your child(ren) can still qualify under a separate eligibility.

    And if you don't have children, most states won't let you qualify for Medicaid at all.

      which gives them a lot of say into how the programs are run.

      As I said earlier, there are good parts about Medicaid's cost controls that should be merged into Medicare.  

      Democratic Leaders must be very clear they stand with the working class of our country. Democrats must hold the line in demanding that deficit reduction is done fairly -- not on the backs of the elderly, the sick, children and the poor.

      up to 185% FPL. Children 1-5 133%, children 6-18 100% FPL.

      The 26% you mention refers to those Aged/Disabled/SSI. Their income limit cannot exceed 74% of the FPL.

      Single adults who are non elderly, not disabled, not parents are not eligible at all for Medicaid, unless they are deemed 'medically needy', and then they can only earn up to 18% of FPL. Sucks to be a sick, poor single adult in Texas.
