    ...the lengths we will go to to circumvent the systems that are in place for our protection.  Between 1979 - 81 I worked as a QC inspector on the floor of a steel fabrication plant.  One punch press machine I remember was set up with twin buttons just above head height and about three feet apart that required the operator to press both at the same time to activate the machine.  Operators were constantly wiring one of the buttons down so they could operate the thing with one hand while using the other put the metal blank in place.  All you could do was walk away and shake your head.  

    Well, I suppose one could have reported them to OSHA, but that wasn't considered a particularly good career move.
