    Trust me, they're not that efficient. Lots of crap going on bogging things down, from incompetence to politics to lack of true incentive. They're business looking to make money. The rest is filler.

      and pushing tasks down to the lowest level.

      I had a "health screening" / needs assessment from a person who couldn't have been making much more than minimum wage, and had no idea what I meant by common medical terms, didn't recognize names of widely used drugs, etc. So if they were going to use that to base approval/denial decisions on, it would be a disaster.
