    Unfortunate word choice.  Decades ago the public health and safety community stopped using the term "accident" because it is loaded against preventive action.

    This explosion wasn't a chance event; it was totally foreseeable, like most injury-causing events.  Calling it an 'accident' contributes to complacency and lazy adherence to the status quo that allows such events to occur.

    Like so many other terms in current use ("entitlements" comes to mind, for one), it's up to us to avoid using it.
