The attorney general is required to report data to Congress each year on applications to the FISA court and the outcomes.  The Federation of American Scientists has an archive of those reports from 1979 to 2012 online.

Matt Bernius went to the trouble of building Google spreadsheets with the numbers from those reports and posted them on Outside the Beltway.  The first spreadsheet you'll see is the older data.  Scroll a bit further for the 2000-2012 information.
