    This is a completely different system, not part of ACA exchanges.  They're simply insurance companies that the individual state contracts with.  

    They will automatically receive Medicaid funding or "premiums" every month from the federal government, whether or not the patient actually uses the plan.

    Some states may call them "exchanges" but they're not regulated by the federal government, except to the degree that Medicaid is regulated.  The expansion of Medicaid was the only thing that was included in ACA.  There's no provision to regulate private insurance sold by states.
