    You should not have to pay more than 9.8% of income and quite a bit less depending on your income level.

    And the false economy, in your case based on a weird premise: "I don't have money to loan to the insurance company while they decide what they're going to pay" is silly. Actually you are paying the doctors before they get any insurance. Your copay is not going to the insurer at any time. If it is refundable it should be coming from the Doctor's office.

    Meanwhile you could have a problem that could be treated more simply and cheaply than it would be later.

      to pay for a $100 doctor visit and wait until insurance decides how much I'm reimbursed, I don't go.

      Under my current plan, I know exactly how much I need to pay, and insurance handles the rest. No reimbursement necessary.

      As for problems treated more simply, That train left 10 years ago.
