    Some of the major carriers haven't even offered their exchange plans to many doctor groups.

    They haven't even offered their plans. So, they are restricting access without even trying to get some physicians to accept their already low reimbursement rates.

    They have gone to some hospitals and large groups with "exclusive contracts" for their exchange products at lower than normal reimbursement levels.

    Most people on dkos already know that insurance companies are truly not in the business of expanding health coverage, they are in the business of expanding health insurance profits.

    It's slow change. But look how many more people today are starting to actually understand the true nature of the for profit health insurance system here.
