    ...is that every hospital believes they have to treat every disease in the name of market share.

    Most of the world tends to concentrate the care of very serious conditions at a limited number of hospitals.  

    Sure, it's inconvenient to have to go to a teaching hospital 3 hours away for treatment. But it's hard to be good at everything.  

    Of course, that would involve setting up transportation, sub-acute care, and accommodations financed as part of treatment, and ways to deliver things like chemotherapy near home without the teaching hospital giving up overall management of care.

    But those things sound suspiciously like welfare to the average American.
