    One semi-recent exception was the UK Channel 4 TV film of John Adams' & Alice Goodman's The Death of Klinghoffer (made in 2003), which the singers sang on set to a pre-recorded orchestral soundtrack.  I think the sound then got properly mixed later in the studio, with those on-set voices.
