    to remove profit from the system; if it was, it wouldn't have been included in the industry-written ACA. Insurers can redefine things that used to be considered overhead as care (advertising becomes "patient education), buy drug company or medical device subsidiary and overcharge themselves, or, hell, even just straight up cook their books.

    Profit has to be removed from the equation totally.

    There's a big difference between no provider limits and the extremely skinny networks that many people are facing in the Exchanges. Especially for specialist care.
