    Re Medicaid numbers generally, here's an article that tries to make the case that it is an exaggeration to say that the 4 million Medicaid signups can be credited to the ACA.

    As I read the article, the first thing that came up in my mind was an interview of Mitt Romney back when he was Governor of Massachusetts, explaining that the first thing they did in addressing healthcare was to study the uninsured population.  The discovered that in Massachusetts, a third of the uninsured population was already eligible for Medicaid or some other type of government assistance, but for whatever reason they just hadn't signed up.  So a big piece of Romney care was having a concerted, sustained effort to sign people up for the programs for which they were already eligible.  

    To put it a different way, a third of the truly needy people were falling through the cracks for lack of effective outreach.  Maybe that was just Massachusetts, but I doubt it.

    So, I say full speed ahead.  You get the behavior that is measured and rewarded -- so the Medicaid signups should be tracked and celebrated.  Later on, the perfessors can go back and overlay the data on prior years' data to see the delta.  For now, sign these people up.
