The suburbs of Cleveland, OH are very blue, even the lily white ones.  Get out of Cuyahoga county, though, and it turns red quick in the outlying counties.

Columbus suburbs are very red, but that's because they are more like exurbs.  The "inner ring" suburbs of Columbus have been incorporated into the city proper for some time.  They bribed the old suburbs with city water and sewer lines to incorporate.

In most places, the farther you get out of the city, the redder it gets. The true rural areas, though, are often split in swing states at least.

different. I grew up in/outside a city on the fringe of the metro area, just beyond the reach of the public transit system, just outside the limits of the metropolitan urban planning jurisdiction, one of the only cities in the county Obama lost even in 2008, while most of the others are suburbs. I know the difference.
