    When doctors have in-house facilities they make money off of every lab they run - fee-for-service. Many, many studies have shown that when these sorts of facilities exist, doctors order more tests (it's convinenient, they get money, why not?), but patient outcomes are not that much better off.

    This is a kind of loophole in the Stark Act which makes it illegal for the doctor to refer a patient to a service they have financial interests in.

    I think health insurance companies have a legitimate concern as there is a conflict of interest. Think about it. Why would a for-profit company health insurance company not cover in-house lab tests if they really are cheaper and just as effective? Then there are all sorts of other issues about CLIA certification of the lab tests...but I won't get into that.
