how about let's talk about how many people would be covered under Medicaid expansion if only their damn governors would accept free money.

graduate student loans and now the ACA is that the layers of middlemen between the Treasury and the individual citizen have been considerably reduced.
Since our commercial class is made up of people who depend on claiming a share of the assets of both the producers and users of goods and services and chambers of commerce are the primary conduit for sending "representatives" to Congress and other legislative bodies, the commercial class quite rightly perceives a challenge to its very survival. Capped overhead and profit margins for insurance companies doesn't help for, when insurers collect less, there's less money to funnel through to the banks and mortgage lenders.

When the distribution of dollars to citizens is more direct, the middlemen get less.

Commerce has led a storied existence. Most economists, being focused on production and consumption, largely overlooked the role played by the legalized highwaymen, who take free (or cheap) stuff to market for a hefty profit without adding any value.
Keep in mind that the traditional merchant's markup was 100%. So, even when they reduced prices on the left-overs from a season or shop-worn stuff to 50%, they still got back what they'd paid out. It was easy for WallMart to lower prices and undercut the other merchandizers. But, WallMart did not start the race to the bottom. Montgomery Ward and JCPenny and Sears were ahead of them in extorting ever more of manufacturers' profits until they had to move off-shore.
