    If there is a bureaucratic way of making ACA fail, the GOP will use it. They can then use that failure to repeal.

      Though I share your concerns about sabotage by Republicans of ACA.  Problem is, they have thrown their best shots at derailing Obamacare - lawsuits by state AGs, SCOTUS cases, 50+ wasted votes in Congress, a government shutdown, the refusal to accept Medicaid payments, ad nauseum.  And yet, it is up and running.  

      The trend, I think here, is that Medicaid expansion (something that was "optional") will become a force in all those states that foolishly refused it.  The Republican govenors will pay a political price for that ideologically and politically driven denial of benefits for their own citizens.

      There's some old saying "Nothing can stop the force of an idea whose time has come."  It's come.
