    the Skywalks at the Hyatt Regency hotel collapsed on a crowd of people.  Over 100 people died, many other sustained severe injuries. The carnage was unbelievable. The floor was covered in blood and severed body parts. The investigation showed that the welding on these suspension walks was poor, and that the original design plan was replaced by one where the first level  skywalk was attached to the second level skywalk, instead of being suspended from the ceiling beams.

      ....was an example of the contractor not building according to the PLANS, and because the Project Manager didn't ensure contractor compliance.

      The rods suspending the structure were supposed to be single lengths that went completely through the mounting points on the upper structure, then continued down to the lower structure, the contractor used shorter lengths of rod which went down to the upper framework, then drilled additional holes adjacent to the holes specified on the plans and placed separate rods from the upper framework to the lower.  The nuts threaded onto the lower set of rods pulled through the holes, allowing the lower walkway to collapse.

      They didn't follow the PLANS.  Nobody looked to see if they were following the PLANS.
