    1. We don't know if SA-11 system was stolen from Ukrainians (they claim no systems are in place that far East of Kiev.)

    2. We have some anecdotal and pictorial evidence that separatists had the system.  Same evidence exists of high ranking Russian military advisors/leaders in the area of the launch.

    3. CIA/NSA...more likely DOD but even more likely NATO already has the radar evidence that a missile was launched from the Russian/Ukrainian border.  So it's been tracked.  Without boots on the ground, however, you can't hunt individuals down.  NATO was staying out of this conflict.

    4. ICAO is the UN agency in charge of international aviation.  They shut down the airspace about 5 hours after the incident, when it was verified that a missile shot down the plane.  FAA only controls US air carriers.  Malaysia Air is an international air carrier.  FAA issued its own ban to US carriers over Crimea when it was unclear whether Russia or the Ukraine was controlling airspace.  FAA follows ICAO guidelines overseas, but sometimes goes further, when conditions warrant.

    Congress will likely look into why FAA didn't respond sooner to reports of Ukrainian planes being shot down.  Had they issued a warning, ICAO probably would have followed suit.

      ....like you can find in NOTAMS for places like Yemen, are designed to keep aircraft high enough to not worry about MANPADS.

      That's really all you need -- keep above 20,000 ft and have a nice day -- for just about every possibility.

      SAM's like the SA-11 are nation-state grade hardware, not the sort of thing you can score in a Kandahar bazaar.
