pregnancy clinic many years ago I met a woman who had two children post two separate tubal ligations.  Unbelievable. Some people are unbelievably fertile. She had five children when she got the first tubal.

She had a heart condition that caused additional pregnancies to be contra indicated.

Birth control failure rates are much higher than most people know. The likelihood that a woman will have an unintended pregnancy is quite high.

The failure rates of birth control methods are stated as the percentage of women in one year that get pregnant using a particular birth control method. The rate for condoms is 15% per year.

I woman's fertile life may be say 30 years.

If a woman's partner uses a condom every time she has sex over a fertile life of thirty years sooner or later the odds of getting pregnant are likely to catch up with her.

You say she should use a more effective method. That is easier said than done as there are many reasons including age that contraindicate their use.

Here is information about the failure rates of various birth control methods.
