    I concede to your accuracy here - however, I would point out that most of the "A and B" examples you used could be considered idioms and therefore could be considered exempt from grammar rules, just as "To boldly go where no one has gone before," has been tolerated (to the point where grammar experts gave up and said that it's legal in English now to reverse the verb and adverb).
