    In the Lancet study the dead were divided into those killed by Coalition forces, those killed by other Iraqis, and those whose killers were unknown. I believe the largest group were those killed by Coalition forces (well over 200,000) and its reasonable to presume a significant fraction of the "unknowns" were as well. Remember the US-led forces have overwhelming superiority of firepower, including a monopoly on airpower. We don't hear much about those killed by US bombing and other aerial attacks because its neither convenient nor comfortable to report on, but that doesn't mean it isn't happening on a massive scale. It is not unreasonable to believe that US-led forces have directly killed several hundred thousand Iraqi civilians, and of course the US also bears considerable responsibility for having plunged the country into sectarian strife and the resulting Iraqi-on-Iraqi death tolls.

    Of course the situation is chaotic and the US can't be blamed for every single death. But it is self-deluding to deny that the US bears primary responsibility for the lions share of these deaths.
