    are New Hampshire and Vermont.  Each has its own BC/BS company.  Both are non-profit.  In each state, BC/BS pays no premium taxes (all other insurers do) in exchange for accepting all comers for health insurance.  They establish rates for individual health insurance that the states approve, and they're outrageous.  This is the "state managed margin" at work here.

    New Hampshire's BC/BS is owned by Wellpoint.  There have been repeated rumors that they would abandon this sytem, but no move yet.
