a lot of older hospitals built in cities in the 40s and 50s are being closed down and relocated in the suburbs.  This creates real access problems for people without cars.

A public option for healthcare that allowed individuals to buy into Medicare would do great things for American manufacturing.  Many firms in mature sectors face a large number of retirees relative to their current workforce.  One example being the auto industry.  This allows new firms entering the sector a huge, and unfair, advantage.

Once you remove retiree costs from GM's wage structure, the rate benefits and pay for current workers is virtually the same as for Japanese transplants.

Unspoken in the discussion of creating a public option is the simple fact that a number of VEBAs run by unions for retirees in auto and steel.

      should tack on:

Unspoken in the discussion of creating a public option is the simple fact that a number of VEBAs run by unions for retirees in auto and steel will choose the public option.

Meaning that it quickly will become the largest organization in the health care insurance market.

So the market will bring national health insurance, because it is the most efficient option.
