    It's passed every year as a rider to the Medicaid appropriations bill.

        But it won't be easy. There will need to be a sustained public campaign to drive it, since there's no push for it from the top.

        It'll also require a willingness to risk Medicaid funding, since much of the opposition will be glad to hold that hostage.

        On the other hand it's higher profile now than it's been in decades, so now might be a good time to start.

          Add a small exception every year or so that allows a little more coverage, and eventually it will be "as good as" gone.

          Both sides can play the salami game - why do we refuse to learn how?
