    I went to work for the first clinical pharmacists our facility hired in the early 90's.  Vanco was one of the first drugs they evaluated for appropriateness of use.  Unless the physicians are monitored for drug use, they tend to coast and order drugs without concern for the big picture.  If patients are being seen by more than one physician, the problems can multiply, e.g. therapeutic duplication, incorrect dosing, etc.

    The clinical pharmacists also sit on the Pharmacy and Therapeutics Committee.
