     I worked at a company that had very active safety committees at every factory.  This is in the semi-conductor industry where the chemical danger is huge.

     The safety committee at each factory was a line worker from each shift and each major area, the Safety group manager, and the Plant Manager.  They met monthly.  Safety indicators were publicly posted every month, and every workers bonus, from bottom to top, was in part tied to the safety results.

     I also knew an engineer who was walked out the door for hiding in a conference room during an emergency evacuation drill.

    So it can be done right.
