    On June 22, for example, BP engineers expressed concerns that the metal casing the company wanted to use might collapse under high pressure.

    "This would certainly be a worst-case scenario," Mark E. Hafle, a senior drilling engineer at BP, warned in an internal report. "However, I have seen it happen so know it can occur."

    The company went ahead with the casing, but only after getting special permission from BP colleagues because it violated the company’s safety policies and design standards.

    There's your smoking gun. Well, one of many smoking guns.

      I can't tell you how many times I've seen this in management over 30 years. I've done it myself, as late as this morning, in fact.

      You trust that you know what you're doing because you've done it a few times before, and you begin to make assumptions - which amounts to shortcuts - for the simple reason that it's expedient!

      Instead of taking the time to ensure that all your ducks are lined up. I learned that myself, being taught this by having committed every error imaginable. BP, with so much more at stake for everyone, should've damned well taken special pains.

      BP deserves to die over this.
