    is intended to lead by example the rest of the health care system to improve quality and reduce costs.

    Beyond funding increased costs for an increased population of Medicaid patients it doesn't do much with Medicaid which are 52 separate systems. thus Medicaid is not a single payer system. Medicare — for the most part is. The Medicare Advantage plans aren't, nor is the required prescription drug plans.

    And the new law does put mandates on the insurers to cover certain things, to pay out certain percentages, remove lifetime (now) and annual limits on coverage and most importantly to cover all people, not charging more for health status. It also sets some limits to out of pocket payments overall. There are other regulations on insurers.

    Delivery of health care is NOT by insurers. It is by providers: doctors, hospitals, DME providers, pharmaceuticals, therapists of all sorts. THEY are the costs drivers. They are where you have to attack costs and quality.

    You are mixing up financing with provision of care.
