    answer for very complex questions.

    The simplest answer is GREED and SLOPPINESS.

    Now, not all in all the systems  are, and the amounts of both are variable.

    But the American Character extolling individuals maximizing their own interests and the liberty of doing things their own way (blaming others for what goes wrong), gives us the health care system we have. That and out childish insistence on wanting the BEST and wanting it RIGHT NOW.

    We hate regulation, it goes against our "freedom" and "liberty," unless, of course, it protects us. Then it's ok. But the one being regulated screams bloody murder.

    And the health care field is a great illustration of those things.
