      This is what it is.

      It also means that a doctor can get in trouble with Medicare /Medicare for giving out free care.

      Think about it, giving out free care means that someone is getting cheaper service than the government.

      Many doctors do it anyway even though they know they could lose their Medicare license...not an insubstantial risk.  

      Doctor's sometimes get around this regulation by telling the patient that they will be sent a bill, but to pay no attention to it...just don't pay it.  The doctor eats the cost of sending out the bill to a person the doctor knows can not afford his services. In addition to being just plain stupid, it is terribly demeaning to the patient to treat them this way.
