Today I received an email from the McCain campaign encouraging me to vote absentee, because as they say:
Voting early by absentee ballot or in person is the best way to get your vote counted now and avoid the long lines on Election Day.

Having already read about a number of instances of GOP efforts to disenfranchise voters, I thought I'd better see for myself what kind of "help" they are offering.

So it looks to me like this is the same faulty absentee ballot that the McCain campaign sent out to millions of voters in Ohio. Many of these were rejected by Hamilton County because voters did not check (an unneeded) box on the form stating that they were eligible to vote.
I refer you to peruse this diary that cubsfan wrote about this issue awhile ago.
It is terribly disturbing to me that even after potentially disenfranchising thousands of voters in Ohio, J. McCain still has this flawed form on his website. I don't know how to link to a pdf file, or otherwise reproduce it here, but this form does still have the unneccesary box that must be checked for the form to be valid. If you go to his website and try to apply for an absentee ballot there, I think you will find the same thing.
