The following is based on an actual workplace survey, intended to be filled out anonymously and submitted to management for… well for whatever they intend to do with it. Questions have been paraphrased and re-ordered. They are presented here only for discussion and educational purposes. You can't fill them out here, and no one at Kos (to the best of my knowledge) is tracking this. You can fill them out for your own use if you like. There will be no grades. If nothing else, they will make you think about work in ways that normally just get ignored in the press of getting through the day. Think of it as a Big Picture exercise.
Unless otherwise specified, all questions are to be answered Yes or No. The survey begins below the Orange Omnilepticon.
1) During the work day, I generally am in a positive, energized frame of mind.

I'm not going to provide any context for this survey, other than to note: NOW they want to know?!?!!! I'll throw out a few more questions. How would this survey strike you as a worker? A manager? A business owner? What would you do with this information? What do you think is meaningless here, and what is really useful - and what else should be included? And do you think any of this has any connection with anything you've experienced as a worker, manager, or business owner?
