Where will Nader make the ballot?

Does anyone know which states are likely to have Nader on the ballot in this election?  Links to any pages with good coverage of this issue would be appreciated. I think Nader is polling much higher now than he will in November.  

First, there's the getting-on-the-ballot issue.  Second, there's how people will get more involved as they see Kerry vs. Bush heat up in debates and as the race intensifies.  Third, I think the fear of being a 2004 version of the Florida Naderite will intensify as one walks into the polls.  Fourth, Nader may have a name recognition advantage over Kerry in some circles, and that'll definitely change.
