As a psychotherapist, I often look at Trump clinically: “Understanding the dangerous of Trump’s sadistic malignant narcissism” March 16, 2019, Daily Kos, for example.
If Donald Trump was my patient I would consider him dangerous and "pink slip" him for an inpatient evaluation because his aggressive impulsive behavior (words and tweets are behaviors) suggest that he is dangerous to himself or others. Not having him sitting in my office I can look at his tweets as indicative of what is going on in his mind.
The mental health perspective
If I was treating him as an inpatient and was a psychiatrist (I’m a clinical social worker) if he was having a psychotic episode or if he was suffering from a major depressive episode I would most likely medicate him.
If he ended up as an outpatient, I would look at the many stresses he is under, not only those in my illustration but add in the 2020 campaign where not only Democrats but Republican will be attacking him, with possible competition from sane members of his own party like Mitt Romney, Jeff Flake, and Chris Christie. If I had a patient with all these job related stressors I would insist that they quit their job and tell them that if they didn't there was no sense remaining in psychotherapy. No doubt there are other stresses we don't know about, possibly coming from Melania who may be at or near her own breaking point.
The lawyer's perspective
My friend who is a clinical psychologist who does many forensic evaluations and often testifies in court tells me the lawyers she deals with generally have a DSM-5 on their desks. This manual is not only the bible of psychiatry, but it is also the roadmap used to navigate the intersection between psychiatry and the law.
I am pleased and amazed at how it is George Conway who has brought Trump's actual diagnosis of malignant narcissism into the mainstream media. By posting the DSM-5 criteria for diagnosis in Twitter he just doesn't dare people to look up narcissistic personality disorder, he posts the definition, and that is only part of malignant narcissism. The other part is anti-social (sociopathic) personality disorder. For good measure, he posts that definition too.
The 25th Amendment is the only way to “pink slip” a president
There is no way to “pick slip” a president. The only avenue for assuring he gets the treatment he needs is through the 25th Amendment. There is no way this will be invoked unless he has a blatant psychotic episode where he is so obviously detached from reality that even his supporters can’t deny it.
Consider this. If Trump’s tweets of the past few days were expressed orally in a psychiatrist’s office along with the emotions that seem to be associated with them, and in the random order they were tweeted, what do you think the doctor’s next step would be?
