The episode opened with Cartman waking his mom up to go purchase a PSP the morning they came out. When he gets to the store, he finds a massive line, with Kenny at the front of the line. Cartman says to himself that they better not run out.
The next morning, we see Kenny playing a game of "Heaven versus Hell" on his PSP. Cartman is infuriated that he couldn't purchase one, and curses at Kyle. Kenny spends the next 2 weeks playing the game constantly, and moving up to higher levels. Eventually, his mother complains that he's spending all his time playing that game, and if he died the next day he'd have nothing to show for his life but that game.
Kenny then finishes level 60, the final level, walks outside, and is run over by a truck.
Kenny's soul ascends to Heaven (again) only to discover that Heaven is in crisis. Satan is planning an attack with his army of tens of billions, and Heaven only has an army of about 10,000. It is then revealed that God created the PSP to find someone on Earth who could figure out a way to defeat Satan's army, and Kenny was the only one to win the game. Kenny is declared to be Heaven's "Keanu Reeves".
As Satan prepares his army to depart (in a fairly exact duplicate of Saruman's speech in The Two Towers), he learns that Heaven has found itself its Keanu Reeves, and Satan decides to call off the assault.
At this point, Kenny's soul suddenly disappears from Heaven. We then learn that a doctor has found a way to revive Kenny, but his brain is completely dead, and he is in a PVS and is being fed by a feeding tube. The angels in heaven are outraged that the boy is being kept alive artificially. Satan learns of this, and sends his army marching to Heaven.
Back on Earth, we see that Kenny left a will, leaving everything he owns to Kyle and Stan, except his PSP, which goes to Cartman. Kenny also spelled out that if he were ever in a vegetative state...and then the lawyer lost the last page. The lawyer and the boys then learn that Kenny has been revived, and is being kept alive on life support. They go to see him, and his mom says she thinks he smiled at her. He doesn't move. At this point, Cartman starts a campaign to get the tube removed, so that he could get his hands on Kenny's PSP. Stan & Kyle, on the other hand, decide to try to save Kenny, and decide to get media attention.
Within a few hours, the nation's media are focused on the town, as protestors cry out to save the boy. Cartman meanwhile goes before a judge and proclaims that Kenny and he were "BFF" (Best friends forever) and that he should be able to make the decision. The judge agrees with Cartman, and the tube is removed.
In an effort to stop the feeding tube to be removed, Satan's current boyfriend (a demon who really resembles Emperor Palpatine) suggests that they should take action to try to keep the boy alive so that his soul can't fight for Heaven. Satan asks what they can do. The other guy responds "Simple. We'll do what we always do. Use the Republicans".
We then see President Bush making a speech calling for Kenny's life to be saved, with that demon whispering all of his words to him.
As the protests intensify, they move into Kenny's hospital room. And then...just as everything was growing louder, the lawyer found the last page of Kenny's will. His wishes were..."If I'm ever in a vegetative state, for the love of God don't put me on national TV." Kyle and Stan realize that while Cartman was trying to remove the tube for the wrong reasons, and they let Kenny die.
Kenny returns to Heaven in time to defeat Satan's army. He is rewarded with a golden statue of Keanu Reeves.
