A thought occurred to me tonight (and if I'm not the first to blog on this, I apologize). We've all seen segments on "The Daily Show" or blog entries pointing out how Republicans go on camera and spout the same talking points, almost verbatim. Whether it was the case of Scott McClellan's memoir leaving GOPers "sad" and "shocked", or such classics as "cut and run" or "up or down vote", Bush's mouthpieces could be counted on to come out and make identical statements of support which often parroted the same language used by the president himself.
One great way to hammer home the idea of McCain being an extension of Bush would be to look for clips of McCain appearances on Fox or Hannity or whatever, where he's making statements identical to Bush's and (if such instances exist, which I'm assuming they do) then put the two statements in a commercial: First Bush making a statement and then McCain saying the exact same thing. I think spots like these would go a long ways towards getting the message across to the voters.
