Well then click on the enroll now button. It includes name,address,date of birth,gender date and total monthly gross income. How did you hear about us? And are you diabetic? Are you on Medicare? The form has lines for 15 different medications-strength-times per day and Doctor's Name and Address. The number of meds x $5 = total you pay for the meds. There is no limit to the number of scripts.
They will send you a packet which you take to your doctor where you prodeed with the application process which if approved is sent to the appropriate sponser. In 2 to 3 weeks the meds are sent to your doctor to be dispensed. You get a 90 day supply. The application fee is $10 which is refunded if you don't qualify.

They are now pushing something to the poor masses rather than letting just a few people hear about it. They will do anything to stop universal health care and this takes some of the teeth out of our demand.
